The Diabetic GP Clinic is a novel GP primary care clinic consisting of a multi-disciplinary team of doctors and allied health practitioners under the one roof, all with the key focus on the prevention and care of diabetes and weight management. It operates by delivering an evidence-based, multi-disciplinary team management approach to the prevention and care of diabetes, weight management and other diseases caused by or associated with obesity. Unlike the current referral-based system which relies on patient compliance, service availability and cooperation between numerous external health professionals, The Diabetic GP Clinic will provide patients with the convenience of the relevant allied health and primary care services all under the one roof. The Diabetic GP Clinic will comprise of the following health professionals:- - Four General Practitioners; - Nurses; - On-site Pathology; - Diabetes Educator; - Dietitian; - Exercise Physiologist; - Psychologist; and - Podiatrist. As a type 1 Diabetic, Dr Kenny Clark is passionate about diabetes and medically-led weight management. With type 2 diabetes being the world’s fastest growing lifestyle disease, Dr Kenny’s goals are for The GP Diabetic Clinic to:- - Raise the level of diabetes care and education within the community; - Improve the quality of life of those diabetes (whether type 1 or 2)
