Description

In November 2011 Keiro announced the development of a new service in Middlesbrough’s Middlehaven district. ‘The Gateway’ will provide a range of new services to people with neurological condition at all stages of their individual pathways, from expert in-house rehabilitation and specialist housing to health and wellbeing facilities for the whole community. The Gateway is a new concept in providing flexible, support along each individual’s unique pathway to independence. This flexibility is facilitated by offering a number of rehabilitation, specialist care, support and housing options, targeting the right support when the client needs it, avoiding delays in accessing services and linking in with partners who will work with us to deliver holistic support. There are three main areas of service that the gateway will provide: a state of the art rehabilitation centre, community wellbeing hub with an integrated health club and a range of transitional housing options on site, with access to long term housing in the wider geographical area. Underpinning principals: To provide the right support, targeted at the right time. Supporting the individual at all stages of their pathway. To provide opportunities for clients to meet their own personal outcomes, saving resources as they gain greater independence. Partnerships: working together with a range of stakeholders to support people holistically and build community capacity Long term independence pathways: housing and support at home for better outcomes and better value in the long term Helping people with long term conditions to stay healthy and live independently
