Clinic Hours: Monday - Friday, 9:00 a.m. - 5:00 p.m. Thursday Evenings We are not a walk-in clinic - Patients must make an appointment. How to apply for services - Eligibility Screening for Clinic Services: Mondays and Wednesdays, 1:30 p.m. - 3:00 p.m Thursdays, 5:30 p.m. - 7:00 p.m. No Appointment Needed for Eligibility Screening Clinic Services: - Acute Care - Primary and Chronic Care/Nurse Practitioner Daytime Visits - Diabetes Management - Gynecology and Women's Health Care - Orthopedic Care - Limited Behavioral Health Care - Referrals to Specialists - Medication Assistance (4-6 weeks enrollment) - Referrals for Lab Work and Diagnostic Testing
