In November of 2012, Massachusetts passed Proposition 3: "The Massachusetts Medical Marijuana Act", This bill protects patients with debilitating conditions from legal prosecution for using marijuana. Qualifying patient's must be seen by a doctor & have the physician recommend the use of medical marijuana to treat their specific condition. Patients must also register with the Massachusetts Department of Health Services and pay them an annual fee of $50. Upon doing so, patients will receive a Medical Marijuana Card, after which they are permitted to use, posses, grow and purchase medical marijuana. To become certified as a patient in Massachusetts you can follow the instructions below. Step: 1 Your Doctor at Green Cross Massachusetts will need to review your medical history. Please gather your recent medical records, along with any related records pertaining to special conditions. Please send your records to us prior to the scheduled date of your appointment. Doing so will ensure that physicians can review your records prior to your meeting. This keeps any wait times to an absolute minimum. If you need assistance obtaining your medical records, complete the form “Medical Records Release”. Send us the completed form and we request the medical records on your behalf. Step 2: After receiving your medical records, our physicians will evaluate and review them. If we require any additional documentation, we will contact prior to your appointment. Step 3: During your certification appointment our physicians will speak with you regarding any conditions that may qualify you for the Massachusetts Medical Marijuana Program. After your review and approval, qualified patients will receive a signed Physician Certification Form to be used for application to the state. This letter will be your temporary Medical Marijuana Patient Card, and is all that you will need for now to use, posses & cultivate Marijuana under the states new Marijuana program. Please bring the following with you to your appointment: 1. At least one year of your medical records. 2. A state or government issued ID 3. Payment of $189 in cash or Credit Card. (VISA/MC/DS/AMEX)
