Homecare Support Plymouth is the domiciliary care and Reablement division of the Human Support Group based in Plymouth.

Description

The aim of reablement is to provide the support a person needs to readjust to living at home, for a period of up to 6 weeks, and enables them to live a confident, independent lifestyle with as little support as necessary when the reablement period ends. HSG Reable provides help and assistance to individuals when they leave hospital or respite care after an injury or illness, helping them regain their independence and stay safe during their recovery. To make sure we provide a service that is meaningful and purposeful to the individual, we assist them in setting their own personal goals for their care, we help the Service User achieve them through a reablement plan that is individually tailored for them, and reduces the amount of support they need over time as their health improves. We provide our services 365 days a year, 7 days a week from 7am-11pm, so we can provide the service a Service Users needs, when and where they need it. Our Service Users are referred by Local Authorities, and are individuals who have been highlighted to have the potential to improve and regain their independence.
