For on-site services, reduced fees are available to those with no health insurance and with incomes below federal guidelines. Presumptive eligibility for Medicaid offered on-site (if your eligible for Medicaid you will be able to use it immediately). Off-site services, including pharmacy, laboratory and hospital services are the patient's responsibility. To assess payment status, proof of income is required and, if applicable, verification of tribal descent. Proof of Income: Paycheck stub - W-2 or tax return - SRS programs - Worker's Compensaation - Social Security - Veterans benefits - Self-employment - Pensions - Retiremen t - Unemployment - SSI - Child Support - Alimony Proof of tribal descent: Tribal enrollment card - Tribal voter registration card - Certificate of Degree of Indian Blood (CDIB) - Bureau of Indian Affairs correspondence - Birth certificate if it documents heritage Our Native American registrar assists patients in obtaining documentation.
