Interfaith House staff and volunteers support each of the resident’s holistic recovery by providing a wide range of opportunities including: • A front desk manager staffed 24/7 • Assistance with medication management • Three fresh meals a day • Daily transportation to appointments • Donated clothes and toiletries • Educational sessions • Daily Living Skills classes • Recreational activities • Spiritual services • Behavioral Heath Counseling • Psychosocial Assessments

Agency Programs On-site Care Complicated, long-term health issues make it difficult for many homeless people to transition into stable housing. Our Programs focuses on providing clients with the opportunity and resources to better equip themselves for life after Interfaith House. Assessment Respite The goal of our Assessment/Respite Program is to help ill or injured homeless people complete a medical recovery plan, receive support with social and psychological issues and to obtain successful placement into housing or further treatment. Admission to the Assessment/Respite Program is by referral from nearly 40 area hospitals and via the Chicago Department of Human Services. Assessment/Respite residents are assigned a case manager upon arrival at Interfaith House. Each case manager works closely with their client to assess their needs and direct them towards the services necessary to support their recovery. Housing Advocacy Team Our Housing Advocacy team determines the most suitable housing for clients after leaving Interfaith House. They develop partnerships with housing providers in Metro-Chicago to ensure a safe living situation for our clients. The Health and Housing Outreach Team Our Health and Housing Outreach Team provide case management to successfully discharged clients who are in stable housing. By providing intensive off-site care to our former residents we ease their transition into mainstream society. These wrap-around services lessen recidivism and prevented over 80% of our participating clients from returning to homelessness
