Today was another interesting day. I spent 25 minutes on the phone trying to get a prior authorization on a generic 4 dollar medicine for a Medicare Part D patient thru their mail order pharmacy. The medicine you ask ? Elavil! It is on the 4 dollar generic list at a local pharmacy. Why bother? I thought it was a mistake at first when it was denied. Cheap, effective and would help with three conditions a patient is suffering from. So I called the Prior Authorization line, after answering a barrage of questions it was still denied. I insisted on speaking with the pharmacist and did and was again denied. But if I wanted to write for three separate name brand medications, one to treat each condition for a total of 600 dollars a month they would approve it without a prior authorization. We called the patient and they agreed to pay the twelve dollars at the pharmacy they took it to. So much for my attempt at cost containment and trying to minimize how many medications a senior is on.
Has any one else noted the changes in generic medications? Today I overlooked the formularies for Wal-Mart and HEB and have noticed many old drugs particularly antibiotics have fallen off these list namely erythromycin and doxycycline. I used the app goodrx.com and was floored to see what the cost for doxycycline is now 250.00, erythromycin 250mg is 150.00. So much for cheap generics and affordable meds.
