Description
NNDHP is... a program which provides dental care to qualified children up to the age of 18yrs old, children who live in Northern Nevada, children with Medicaid or NV Check Up, children with no dental insurance and whose family meets program income guidelines. Services include: •Preventive oral health care including: education, fluoride varnish, dental sealants and teeth cleaning. •Comprehensive restorative oral health care including general dentistry and specialty care. •English and Spanish language capability for better communication. Dentists (volunteer), utilizing their own offices provide treatment and prevention services to patients. After qualifying for the program, children are scheduled into needed care.

General Info
•The following must be submitted with your application annually for each child: proof of income, proof of residence and proof of household composition. •All families who are not covered by Nevada Medicaid or Nevada Check Up must pay an access fee of $300. •Families must attend an oral health/nutrition class prior to treatment and agree to have the child checked by a dentist 6 months post treatment.Company Rating
