We have been an anchor in North Minneapolis for 42 years, providing comprehensive, personalized care to our patients. Our clinic is part of UNIVERSITY OF MINNESOTA PHYSICIANS, the multispecialty practice group for University of Minnesota Medical School faculty. Our patients range in age from newborns to seniors. HEALTH CARE SERVICES include physicals, care for acute or chronic illnesses, preventive medicine, prenatal care, adolescent medicine, sports medicine, behavioral health, addiction medicine, and more. Our clinic is certified as a HEALTH CARE HOME, which means we take a collaborative, team-based approach to patient care. This improves health outcomes and quality of life for our patients. We have integrated BEHAVIORAL AND MENTAL HEALTH services into our practice to better serve our patients as a whole person—mind, body, and soul. Our patient care teams develop PERSONALIZED CARE PLANS that meet each patient's needs as an individual. We strive to remove any barriers to patient care. With our connection to the UNIVERSITY OF MINNESOTA, we are dedicated to innovation and leadership in the field of family medicine. We teach tomorrow's physicians how to provide the best care for patients and communities. COMMUNITY OUTREACH is also an integral part of who we are. Outreach activities include weekly clinics at local homeless shelters, a mentorship program for youth interested in health careers, an education series exploring the social determinants of health, training room and sideline coverage at local high schools, and more.
