Founded in 1889, Visiting Nurse Association (VNA) of Colorado is the Rocky Mountain Region’s oldest and largest home health care agency. A nonprofit agency, VNA improves quality of life by providing excellent health care to people where they live and work. Exploring and developing health-related programs based on community needs, the agency provides an array of services in three focal areas: home care, hospice-at-home and wellness services.

We provide the following continuum of health care in the home and community: COMMUNITY WELLNESS SERVICES VNA wellness clinics provide a variety of preventive services at community locations, workplace settings and senior centers. Wellness services include cholesterol and glucose screenings; blood pressure checks; medical foot care; TB skin tests and immunizations for flu, tetanus, pneumonia and Hepatitis A & B. Nurses monitor chronic conditions, such as heart and lung disease, diabetes and arthritis, and provide education on medication management, maintaining healthy lifestyles and in-home safety. HOME CARE VNA medical professionals develop individualized, comprehensive care plans for patients enabling recovery at home from an illness or surgery. Physicians, nurses, social workers and therapists – physical, occupational and speech – collaborate to assure patients have positive home care experiences. Insurance companies require physician referrals for home care from these professionals. VNA also has private pay services for companionship and personal care in the home. HOSPICE-AT-HOME VNA offers care in the home for those with terminal illnesses who can no longer benefit from curative treatment and have life expectancy of six months or less. Every hospice patient is cared for by a VNA team, which includes nurses, certified nurse aides, social workers, chaplains, dieticians and volunteers. This team collaborates with the patient’s physician to provide the physical, emotional, mental and spiritual care that are especially important at the end of life. VNA hospice services are fully covered for Medicare beneficiaries as well as for many patients with private insurance plans.
